Fields you need to submit:

Serial Number - data is required in this column for all records
User First Name - data is required in this column for all records
User Last Name - data is required in this column for all records
Address 1 - data is required in this column for all records
Address 2 – this is optional
City/Town - data is required in this column for all records
State/Province - data is required in this column for all records
Postal Code - data is required in this column for all records
Country (must be a 2-character ISO2 country code) - data is required in this column for all records
Phone Number (numeric values only) - this is optional
Email - this is optional
Has Ozone or Activated Oxygen been used to Sterilize the Device? - this is optional
